How to submit vaccination records to Toronto Public Health [gWJ8WDYaEuJ]

This video shows how to report student vaccines to Toronto Public Health through Immunization Connect Ontario (ICON) at toronto.ca/StudentVaccines. Under Ontario’s Immunization of School Pupils Act (ISPA), students must be vaccinated against nine diseases (Measles, Mumps, Rubella, Diphtheria, Tetanus, Polio, Meningococcal, Pertussis, and Varicella) or have a valid exemption on file with Toronto Public Health to attend school. Vaccination is important for the health of each student and the school community. Students 16 and older can access and update their immunization records online.
